For initial counseling clients, please complete forms 1-4.  For annual consent renewal, please complete forms 1-3 only.  #5 is for your reference.  #6 is optional, only if you want PhoenixWay to share your PHI with others.  #7 is for private pay counseling & coaching clients only.  Copies of completed forms will be sent to your e-mail address.
