PhoenixWay PMA Agreement. This Agreement is made between the PhoenixWay Private Membership Association (“Association”) and the undersigned individual (“Member”). The Association is a private, unincorporated contractual Association owned and operated under the authority of PhoenixWay to Holistic Natural Health. 1. Purpose. The Association exists to provide its Members with private education, counseling, natural healing services, wellness support, and related activities in the private domain. 2. Private Contract. By joining this Association, the Member agrees that all interactions, services, and communications with the Association are private contractual matters. The Member voluntarily chooses to conduct all business with the Association in the private capacity. 3. Membership. Membership is private and by invitation only. The Association reserves the right to accept or reject any applicant and to revoke membership at any time with or without cause. 4. Waiver of Public Rights. The Member understands and voluntarily agrees to waive any rights, benefits, or protections that may exist under public law or statute in exchange for the private services and education provided by the Association. 5. Non-Liability. The Member agrees not to hold the Association or its Trustees liable for any loss, injury, or damage related to their participation in Association activities. 6. Governing Law. This Agreement shall be governed by the common law of contracts and equity as recognized under the Constitution of the United States of America and the State of North Carolina. *

Member Acknowledgment. By submitting this form, I attest that I have read and understand this Agreement and voluntarily choose to become a Member of the PhoenixWay Private Membership Association.
